Welcome to Chi Med Psych PLLC. We look forward to addressing your emotional health needs. Our office policies are designed to create an effective and efficient healthcare experience for you. We recognize that your time and resources are valuable, and we will honor those as part of your complete health care plan. We ask that you acknowledge and abide by our policies. Your partnership in a mutually respectful relationship is very much appreciated. We encourage your questions and participation in all aspects of your care.

Emergencies

If you are experiencing a true emergency or crisis that may put you or others at risk for harm, it is your responsibility to go to the nearest emergency center or call 911 first. Under situations that do not put you at risk for harm, but where you are unable to reach us, it is also your responsibility to go to the nearest emergency center or call 911.

Cancellations

When an appointment is scheduled, the time is reserved for you alone. Therefore, you will be charged the current no show fee, which is $50.00 per session when you don’t show and/or are not cancelled within 24 hours of the scheduled session. This charge WILL NOT be paid for by your insurance company. You are personally responsible for this payment in full. You MAY not be permitted to schedule another appointment until your balance is met in full. Note that multiple no-shows and cancellations in less than 24 hours may result to service discontinuation.

Pricing/Payments

An initial diagnostic/psychiatric assessment/evaluation is billed at a rate of $195.00. Follow up evaluations are billed at a rate of $95 for 20-30 minutes and $110 for 45 minutes. Certain services, such as filling out forms on your behalf, writing letters, court or legal testimony, and other activities that may be required in conjunction with your treatment, will incur an extra charge that will be disclosed to you prior to the time that these services are performed.

Chi Med Psych is credentialled with the following insurances: Aetna; Blue Cross and Blue Shield of Texas; Cigna Optum: UnitedHealthcare, Oxford Health plans, UMR, Oscar, UHC Student Resources, All Savers UHC, Harvard Pilgrim, Meritain, Nippon, Health Plans Inc, Surest (Formerly Bind) *
While we bill your insurance for the services provided, you will be responsible for the co-pay and/or deductible at the time of service as well as for any balance not ultimately paid by the insurance company. Our billing services will submit claims on your behalf. Please understand that your health insurance is a contract between you and your insurance company. Therefore, you are responsible for payment of services which your insurance denies. Please understand that it is your responsibility to keep track of the number of visits, authorizations (if your insurance company requires them), plan limitations and yearly and lifetime maximums. Also, it is important to know that your insurance company represents a third-party interest and may access your files. We will take all allowable precautions to protect your privacy.

At Chi Med Psych PLLC, we require keeping your credit or debit card on file as a convenient method of payment for the portion of services that your insurance doesn’t cover, but for which you are liable. Please understand that we keep your credit card on file. Your credit card information is kept confidential and HIPAA secure. You can go to the patient portal and add a credit card to have on file yourself or we are happy to do it for you.

We are committed to protecting your privacy.  So, we secure the personal health information you provide on computer servers in a controlled, secure environment; protected from unauthorized access/use or disclosure. When personal information such as a credit card number is transmitted to other Websites, it is protected using encryption, such as the Secure Socket Layer (SSL) system. Hence, we secure your personal information from unauthorized access, use or disclosure.

Third Party sites are not under the control of this practice nor are we responsible for the contents/changes/updates of any third-party Site.
